Note that, unfortunately, we do not have the resources for the matching process to be carried out externally, and so your form will be reviewed by Laura Poots or Marika Lemos. Unless a mentee requests otherwise (either in their initial email or at a later date), completed forms will be retained for 2 years after a mentee has been matched with a mentor. The information on this form will be used only for the purposes of matching mentors and mentees and administering the mentoring scheme, subject to any professional reporting obligations.
